Direct a high-performing home care department ensuring workflows and policies are followed at all times
Guide and maintain home care staff, including hiring, training, and coaching all team members
Assess the effectiveness of home health nursing assessments and IDT participation
Ensure nurses deliver and document interventions per participants' care plans

🎯 Requirements

Bachelor's degree preferred; years of experience may substitute
2+ years nursing experience in home health with frail/elderly population
California RN license required
Knowledge of nursing for frail elderly participants and complex cases
