- Travel to patients' homes in Greater Denver and Northern Denver areas for onsite visits.
- Conduct up to 10 virtual/telephonic care plans daily and onsite visits per region.
- Perform monthly visits to ensure quality service and address changes in condition.
- Support PreCare and Care divisions with care plan development and updates.
- Collaborate with the team to provide compassionate, patient-centered home care.
🎯 Requirements
- LPN – Licensed Practical Nurse; Colorado Board of Nursing license.
- Valid driver's license with insured vehicle; travel within CO.
- Minimum 2 years clinical exp, 1 year with elders/disabled.
- Bachelor's Degree in Healthcare Administration (Ideal candidate).
- Knowledge/experience in developing Care Plans.
- CRM/Salesforce experience preferred.
