- Oversee pre-enrollment assessments; collaborate with MOE; attend weekly meetings
- Manage direct reports: hiring, training, supervising and mentoring
- Spearhead internal investigations between Home Care Assistants and participants; work with HR on resolution
- Lead daily meetings with HCCs to discuss pending assessments and hospitalizations
- Conduct check-in sessions with HCCs to complete case reviews and IDT meeting coaching
🎯 Requirements
- Healthcare licensure or equivalency, with 3 years of professional experience
- Bachelor’s degree preferred
- Minimum of 3 years of case management or home nursing with frail elderly
- Must obtain CPR and First Aid within 180 days of hire; certification upon hire preferred
