Review and audit Participant Care Plans by HCCs; coach on documentation for compliance
Oversee pre-enrollment assessments; collaborate with MOE; attend weekly meetings
Manage direct reports: hiring, training, supervising and mentoring
Spearhead internal investigations between Home Care Assistants and participants; work with HR on resolution
Lead daily meetings with HCCs to discuss pending assessments and hospitalizations
Conduct check-in sessions with HCCs to complete case reviews and IDT meeting coaching

🎯 Requirements

Healthcare licensure or equivalency, with 3 years of professional experience
Bachelor’s degree preferred
Minimum of 3 years of case management or home nursing with frail elderly
Must obtain CPR and First Aid within 180 days of hire; certification upon hire preferred
