Review and audit Participant Care Plans; coach HCCs on docs for compliance in Wellsky Personal Care and Athena
Oversee pre-enrollment assessments; collaborate with MOE team; attend weekly meetings
Manage direct reports: hiring, training, supervising, mentoring
Lead investigations between Home Care Assistants and participants; coordinate with HR on outcomes
Lead daily HCC meetings on pending assessments and hospitalizations
Conduct check-ins for case reviews; coach on conversations; support IDT solutions

🎯 Requirements

Healthcare licensure or equivalent with 3+ years experience
Bachelor’s degree preferred
3+ years in case management or home nursing for elderly
CPR and First Aid within 180 days; certification preferred at hire
