Coordinate calls with participants, physicians, and services about orders and referrals
Communicate with participants by phone; coordinate with nursing, therapy, and physicians on updates
Collaborate with Home Care Services staff to track delivery and scheduling hours
Partner with Marketing to support enrollment of prospective participants
Assist with staffing and scheduling activities, seeking manager input
Participate in end-of-life care coordination and support

🎯 Requirements

Healthcare/medical licensure or equivalency with at least three years of experience
Bachelor's degree preferred
Minimum of three years in case management or nursing in clinical/home with frail elderly
Nursing knowledge to treat frail elderly patients with complex conditions preferred
