Home Care Coordinator

📋 Description

• Conduct in-home care assessments for participants.
• Coordinate personal care with the home care team and IDT.
• Communicate with participants, nursing, therapy, aides, and physicians.
• Track home care delivery and scheduling hours with the team.
• Support enrollment and staffing; participate in end-of-life care.

🎯 Requirements

• Healthcare licensure or equivalent; 3+ years experience.
• 3+ years case management or nursing in home/clinical settings for frail elderly.
• Bachelor’s degree preferred.
• Nursing knowledge for frail elderly and complex conditions preferred.
• Strong communication and collaboration with IDT and participants.
• Experience with staffing and scheduling activities.
