Home Care Coordinator

📋 Description

• Conduct in-home care assessments and coordinate care plans.
• Schedule and oversee home care delivery with IDT meetings.
• Coordinate calls with participants, physicians, and agency services.
• Collaborate with nursing, therapy, aides, social services, and doctors.
• Support enrollment of prospective participants with Marketing.
• Assist with staffing, scheduling, and end-of-life coordination.

🎯 Requirements

• Healthcare/medical licensure or equivalent plus 3+ years experience
• Bachelor’s degree preferred
• 3+ years case management or nursing in clinical or home settings with frail elderly
• Nursing knowledge to treat frail elderly with complex conditions preferred
