Conduct in-home care assessments for participants.
Coordinate calls with participants, physicians, and agency services on orders and referrals.
Communicate with participants by phone and coordinate with nursing, therapy, aides.
Collaborate with Home Care Services staff to track delivery and scheduling hours.
Partner with Marketing to support enrollment of prospective participants.
Assist with staffing and scheduling, soliciting input from managers.

🎯 Requirements

Healthcare/medical licensure or equivalent with at least 3 years of professional experience.
Bachelor’s degree preferred.
3+ years of case management or nursing in clinical or home settings with frail/elderly population.
Nursing knowledge and training to care for frail elderly participants preferred.
