Coordinate calls with participants, physicians, and agency services.
Communicate with participants by phone; coordinate with nursing, therapy, and aides.
Collaborate with Home Care staff to track delivery and scheduling hours.
Partner with Marketing to support enrollment of prospective participants.
Assist with staffing and scheduling activities; solicit manager input.
Participate in end-of-life care, coordination, and support.

🎯 Requirements

Healthcare licensure with at least 3 years of professional experience.
Bachelor’s degree preferred.
Minimum 3 years of case management or nursing in clinical/home settings with frail/elderly population.
Nursing knowledge for frail elderly participants and complex conditions preferred.
