Coordinate calls about participants, physicians, and referrals
Communicate with participants; coordinate with nursing, therapy, aides, social services, and physicians
Track and monitor home care delivery and scheduling hours
Partner with Marketing to support enrollment of prospective participants
Assist with staffing, scheduling, and manager input
Participate in end-of-life care and support

🎯 Requirements

Healthcare licensure or equivalency with 3+ years experience
Bachelor’s degree preferred
3+ years of case management or nursing in home/clinical settings (frail elderly)
Nursing knowledge to treat frail elderly and complex conditions (preferred)
