Home Care Coordinator

📋 Description

• Handle calls about participants, physicians, and referrals.
• Communicate with participants and care team about schedule changes and results.
• Track and monitor home care and scheduling hours with staff.
• Assist with enrollment of prospective participants.
• Assist with staffing and scheduling activities with input from managers.
• Participate in end-of-life care coordination and support.

🎯 Requirements

• Healthcare licensure or equivalent; 3+ years professional experience.
• Bachelor's Degree preferred.
• 3+ years of case management or nursing in clinical/home with frail elderly.
• Nursing knowledge to care for frail elderly and complex conditions preferred.
