Handle and coordinate incoming calls about participants, physicians, and referrals.
Communicate with participants and care teams about changes in schedule and test results.
Track and monitor home care and scheduling hours with Home Care Services.
Assist enrollment of prospective participants into the program with Marketing.
Assist staffing and scheduling with input from managers.
Participate in end-of-life care coordination and support.

🎯 Requirements

Healthcare/Medical licensure or equivalency; plus 3 years of experience.
Bachelor's Degree preferred.
3+ years case management or nursing in home/clinical settings for frail elderly.
Nursing knowledge and training to treat frail elderly with complex conditions preferred.
