Outreach to eligible patients and enroll in Health Home Care Management.
Coordinate comprehensive assessments and care plans with the care team.
Collaborate with providers, pharmacies, insurers, and caregivers to coordinate care.
Provide direct service to a caseload, facilitate referrals, and connect to local resources.
Conduct crisis intervention when needed and maintain timely documentation.
Perform home visits and ensure confidentiality per Health Homes standards.

🎯 Requirements

Bachelor's Degree preferred.
2 years of Care Management experience.
Able to travel to multiple NYC boroughs (Brooklyn and Queens).
Travel to accompany patients to appointments and meetings.
Strong knowledge of local community resources and care coordination.
Experience with data reporting and outcomes; Salesforce experience.
