Follow RN care plan to provide personal care, household support, and companionship.
Assist with activities of daily living (bathing, grooming, toileting, nail care).
Provide meals, range of motion, and mobility assistance.
Support one-on-one care at home to help adults stay safe and independent.
Travel to clients' homes as assigned.

Willingness to travel to clients' homes as assigned.
Prior home health care welcome but not required; training provided.
Ability to follow care plans and work with a team.
Excellent communication and reliability.
