- Bachelor's Degree preferred
- 2 years Care Management experience
- Travel to Brooklyn & Queens required
- Travel to accompany patients to appointments
- Strong knowledge of local community resources
- Experience with data reporting, documentation, and outcomes
- Strong communication and assessment skills
- Ability to manage high-volume caseloads
