🎯 Requirements
- 2+ years of experience with frail or elderly disabled adults and caregivers
- Field experience in medical or case management (preferred)
- Data input, case review, and note entry; Salesforce a plus
- Hands-on experience in home care (preferred)
- Knowledge of ADL/IADL assessments
- Valid driver's license with insured vehicle for Indiana travel
- Proficiency in Microsoft Word, Excel, Outlook
