- Perform orientations and assess new patients for the program.

- Collaborate with caregivers, patients, and care team; participate in meetings.
- Identify improvements and provide feedback to the team.
🎯 Requirements
- 2+ years working with frail or elderly disabled adults and caregivers.
- Field medical or case management experience preferred.
- Home care experience preferred.
- Excellent verbal and written communication; strong organizational skills.
- Proficient in Microsoft Word, Excel, Outlook.
- Valid driver's license with insured vehicle for MO travel.
- Salesforce experience a plus.
