π― Requirements
- 1+ years experience with frail/elderly adults and caregivers
- 1-3 years medical field experience and/or case management; case notes and reviews
- Ability to exercise independent judgement and identify issues
- Strong verbal and written communication; build rapport with members and caregivers
- Excellent time management, problem-solving, and multitasking
- Salesforce experience a plus; HIPAA compliant
