- Conduct home orientations and manage onboarding paperwork.
- Perform initial assessments for new patients in the program.
- Train caregivers to ensure high-quality in-home care.
- Communicate findings to care managers, caregivers, and other stakeholders.

- Assess procedures, implement improvements, and train caregivers as needed.
๐ฏ Requirements
- 2+ years of experience with frail or elderly disabled adults and caregivers
- Ability to travel within Connecticut
- Excellent verbal and written communication, organizational and time-management skills
- Proficient in Microsoft Word, Excel, Outlook; Salesforce experience is a plus
- Strong critical thinking and problem-solving skills; team-player mentality
- Working knowledge of ADL/IADL assessments; hands-on home care experience preferred
