Lead field staff to deliver client-centered and quality care.
Act as internal case manager and advocate for Habilitation clients.
Conduct person-centered planning, intake, and regular client visits.
Maintain complete, timely documentation in client/home charts and systems.
Liaise between field and office staff to ensure coordinated care and service delivery.

🎯 Requirements

Prior supervisory experience required.
Proven track record of goal achievement and strong interpersonal skills.
PC skills; able to read, write, and communicate in English.
Must complete pre-employment assessments and background checks.
Hawaii: Bachelor's degree or Positive Behavior Supports training.
Hawaii: Monthly face-to-face supervision/observation as needed.
