- Act as internal case manager and advocate for clients in the habilitation program
- Participate in Person-Centered Planning processes
- Collaborate with client, family, Case Managers, Hab Techs to implement and evaluate goals in the Plan
- Make regular client visits to monitor progress and ensure services align with the Plan
- Maintain ongoing communication with Case Managers/Care Coordinators
- Supervise Habilitation Technicians per policy and state regulations
🎯 Requirements
- Bachelor degree is required
- Must be a Qualified Professional with 1 year post-degree I/DD experience
- Prior supervisory experience a plus
- Demonstrated record of taking on increased responsibility
- Ambition to grow and advance beyond current position
- Excellent communication and interpersonal skills
