Grad Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Conduct mobility, ROM, strength, balance tests, and safety assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and revise individualized care plans (manual therapy, gait training).
• Document observations and interventions on a touchpad tablet.
• New grads receive orientation and educate clients on rehab and independence.

🎯 Requirements

• A current Pennsylvania Physical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or CAPTE.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
