Full Time Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, strength, balance, and safety.
• Develop care plans with manual therapy, gait and balance training.
• Continuously assess and revise the PT care plan with nursing.
• Educate clients and families in rehabilitation and safety for independence.
• Document care using EMR on a touch pad tablet.

🎯 Requirements

• PA Physical Therapist license.
• Graduation from an APTA-approved program; transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
