Full Time Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Conduct mobility, ROM, strength, balance, gait, and safety assessments.
• Develop and implement individualized PT plans including manual therapy and gait training.
• Revise care plans and coordinate with nursing on the multidisciplinary team.
• Educate clients and families on rehab activities to promote safety and independence.
• Document observations and interventions using a touchpad EMR tablet.

🎯 Requirements

• A current PA Physical Therapist license.
• Graduate from an APTA-approved program or AMA accreditation; transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
