Make home visits to clients in designated geographic territories.
Evaluate functional status incl. muscle function, endurance, coordination, and ADL/IADL.
Assess cognitive, psychological, and motor aspects; develop intervention plans.
Evaluate home environment for hazards or barriers to independent living.
Identify equipment needs and environmental adaptations.
Educate patients and families in rehabilitative care to promote health and independence.

🎯 Requirements

PA Occupational Therapist license.
At least one year of recent OT experience.
Graduation from an AMA/AOTA-accredited OT program.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational and problem-solving skills.
Solid computer skills; EMR experience preferred.
