Making home visits to clients in designated geographic territories.
Administer medications, wound care, and treatments.
Monitor clients' conditions; report changes to the Clinical or Client Services Manager.
Follow up and document doctors' orders.
Conduct client assessments as needed.
Occasional on-call after hours and weekends.

🎯 Requirements

Active Pennsylvania RN license.
Minimum 1 year of recent clinical experience.
Prior home care/hospice experience preferred but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to cases.
Strong communication and teamwork.
