Our client is a nurse-owned and established Mobile Physician Practice, that provides improved patient outcomes with convenience, comfort, personalized care and flexibility. With a variety of accessible and convenient mobile programs, the client provides an easy and convenient way to access healthcare services, regardless of location or mobility. 

The client provides comprehensive healthcare services through mobile and telehealth programs, focusing on accessibility for patients with chronic conditions or mobility issues. They offer services including mobile primary care, palliative care, wound care, transitional care post-hospitalization, weight management, behavioral health telehealth, and home-based physical, speech, and occupational therapies. Their programs are designed to deliver personalized care, improve patient outcomes, and reduce hospital readmissions, with 24/7 support and coordination with home health agencies and primary care providers.

please note, this position is 100% remote, however physical presence in Nevada (Las Vegas) is required.

Essential Functions

• Conduct clinical assessments of patients via telemedicine, provide treatment protocols, prescribe or recommend behavioural medications and provide medication management 
• Work closely with collaborating psychiatrists where required by state law or independently but within an interdisciplinary team including other nurse practitioners, psychologists, clinical pharmacists and physicians
• Educate patients and families on mental health conditions and treatment options
• Patient progress monitoring
• Accurate and timely completion of patient notes in the EHR
• Provide consultation to facility staff on nursing practice and psychopharmacology including writing orders for medications, as appropriate
• Participate in case reviews, consultations and utilization review
• Respond to urgent calls from the facility and facilitate problem-solving

Qualifications:

• Advanced Practice Nurse Practitioner Certification and PMHNP
• Valid NP license for Arizona or in the process of obtaining it (may be asked to obtain multi-state licensure)
• DEA certificate (Schedule 3) or in the process of obtaining it
• Experience working autonomously in a virtual setting
• Able to meet the credentialing requirements of clients and your assigned facilities
• Able to become credentialed with Medicare and Medicaid, and other payers as necessary
