Provide skilled home health nursing in participants' homes under CMS guidelines.
Coordinate care plans with the Interdisciplinary Team (IDT) to support independence.
Deliver and document nursing interventions per care plans, ensuring safety and meds/tests.
Instruct participants and caregivers on disease process, self-care, and prevention.
Track and monitor home health scheduling with the Home Health Manager.

🎯 Requirements

Graduate of an accredited school of nursing.
Unencumbered RN license required.
Minimum 1 year nursing experience with frail/elderly population; HH agency experience preferred.
Reliable means of transportation.
CPR & First Aid within 180 days of hire; certification upon hire preferred.
