Initial assessments during orientations
Periodic supervisory visits / check-ins for on-care cases
Training caregivers and patients as required per case
Interacting with caregivers and other stakeholders, as needed
Engaging in interdisciplinary meetings with the care managers, caregivers, and patients

🎯 Requirements

2+ years of experience working with frail or elderly disabled adults and caregivers
Ability to travel throughout the New Haven area and surrounding cities
Excellent verbal and written communication, organizational and time management skills
Hands-on experience in home care, preferred
Strong critical thinking and problem-solving skills
Team-player mentality
