Schedule initial home assessments (virtual/in-person) to screen ADL/IADL eligibility.
Develop caregiver emergency backup plan.
Serve as internal expert on waiver onboarding and orientation.
Conduct quarterly in-home assessments for assigned cases.
Ensure compliant annual training for caregivers per waiver needs.
Escalate health concerns to leadership and log issues in CRM.

🎯 Requirements

Bachelor’s degree or equivalent work experience.
Reliable transportation to visit patient homes.
1-3 years medical field and/or case management experience.
Excellent verbal and written communication.
Strong time management and ability to multitask.
Familiarity with HIPAA; willingness to learn.
