Enrollment RN Per Diem

📋 Description

• Lead initial nursing and home safety assessments of prospective participants.
• Act as liaison between IDT and Outreach Team to ensure enrollment continuity.
• Conduct home visits to evaluate eligibility for level of care.
• Introduce participants to the PACE program and coordinate enrollment.
• Collaborate with Outreach Specialist and IDT to deliver a positive enrollment experience.
• Provide welcoming, responsive care team touchpoints for participants and caregivers.

🎯 Requirements

• Active, unrestricted California RN license.
• Associate degree in nursing; BSN preferred.
• 1+ year experience with frail/elderly; SNF preferred.
• CPR/BLS certification or ability to obtain within 30 days.
• Excellent organization and written/verbal communication skills.
• Bilingual: Spanish/Mandarin/Cantonese preferred.
