📋 Description

• Conduct LOC nursing assessments and home safety evaluations to determine PACE eligibility.
• Perform home visits for in-person assessments of participant needs.
• Prepare LOC assessment documentation and submit to state for review.
• Manage administrative tasks and communications with state and central ERD.
• Coordinate with central ERD and IDT; track assessments and LOC submissions.
• Attend center events as directed by supervisor or MOE Director.

🎯 Requirements

• Graduate of an accredited nursing program; BSN preferred.
• Unencumbered RN license.
• Training and certification as Marketing Representative within 30 days of hire.
• Completion of required state certification for LOC assessments.
• Valid driver's license and reliable transportation.
