Enrollment RN

📋 Description

• Conduct in-home clinical assessment visits for enrollment.
• Lead initial nursing and home safety assessments.
• Liaise IDT and Outreach for enrollment continuity.
• Provide a welcoming, patient-focused enrollment experience.
• Perform home visits to evaluate eligibility for PACE care.
• Document evaluations and maintain care records in EHR.

🎯 Requirements

• Associates degree from an accredited nursing school; BSN preferred.
• Active, unrestricted California RN license.
• Minimum 1 year experience with frail or elderly.
• Ability to travel to participant homes.
• Passion for delivering coordinated care to frail elders; strong communication.
• Bilingual: Spanish strongly preferred.
