- Proactively identify participants coming up for Medi-Cal redetermination
- Educate and assist with completing forms and obtaining signatures
- Conduct visits to participants' homes to complete forms when necessary
- Assist with obtaining documents for eligibility determinations
- Complete and file applications with follow-ups to ensure timely processing
- Maintain strong DPSS relationships to ensure compliance with DHCS/CMS
🎯 Requirements
- High school diploma or equivalency
- Hybrid schedule with regular onsite presence at La Quinta Welbe Center
- 3+ years Medi-Cal/Medicaid enrollment experience incl. appeals
- Experience with county DPSS for Medi-Cal enrollments
- In-depth experience with complex Medi-Cal enrollment case management
- Bilingual (Spanish/English) strongly preferred
