Eligibility and Coverage Advocate

📋 Description

• Proactively identify Medi-Cal redetermination cases
• Educate/assist with form completion; obtain signatures
• Conduct home visits to complete forms when needed
• Obtain docs for eligibility determinations/redetermination
• File applications; follow up with participants and agencies
• Maintain DPSS relationship to ensure DHCS/CMS compliance

🎯 Requirements

• High school diploma or equivalency
• Hybrid onsite Modesto/Stockton Welbe Center
• 3+ years Medi-Cal/Medicaid enrollment experience
• DPSS experience: submitting, managing, appealing Medi-Cal
• In-depth Medi-Cal enrollment case management knowledge
• Bilingual Spanish/English preferred; strong interpersonal with seniors
