ECM Program and Field Supervisor

📋 Description

• Supervise 3–5 Community Health Workers; coach daily ops and onboarding.
• Lead 8–10 week onboarding for CHWs; cover field engagement and ECM workflows.
• Conduct case reviews; ensure patient engagement plans and ECM compliance.
• Monitor CHW productivity, encounters, and documentation quality.
• Maintain a small patient panel (~5) to stay connected to field ops and outreach.
• Train CHWs on ECM, Medi-Cal, trauma-informed engagement, and homelessness outreach.

🎯 Requirements

• 3–5 years in street medicine, enhanced care management, homelessness outreach (preferred).
• Strong knowledge of Medi-Cal managed care and health-plan documentation (preferred).
• Field credible with direct operational outreach experience.
• Skilled educator who translates complex workflows into practical instruction.
• Knowledge of ECM documentation standards and interdisciplinary coordination.
• Organized and metrics-driven with cadence in a fast-growing environment.
