ECM Care Coordinator

📋 Description

• Manage a caseload of patients; complete assessments and care plans.
• Build rapport with patients to foster positive behavior changes.
• Coach patients to minimize risks from identified conditions.
• Accompany members to medical and social services appointments.
• Connect members to programs addressing barriers to care and improve compliance.
• Document information from every encounter in designated information systems.

🎯 Requirements

• High School Diploma or equivalent.
• Bilingual English and Spanish strongly preferred.
• 2+ years of CalAIM experience with target populations.
• Ability to work outdoors.
• Excellent oral and interpersonal communication skills.
• Travel 50-75%; valid driver's license and reliable transportation.
