- Provide home health visits to adult and geriatric clients per visit in SC.
- Support Shooting Stars Division with day shifts, Mon-Fri; no weekends.
- Travel to Columbia, Greenville, and Rock Hill service areas.
- Follow care plans; administer meds; wound care and treatments.
- Monitor clients and report changes to Clinical/Client Services Manager.
- Document observations and orders; coordinate care via a touch-pad EMR tablet.
🎯 Requirements
- Active SC RN license
- Minimum 1 year clinical (medical/surgical) exp
- OASIS and Medicare home health experience
- Graduation from accredited nursing program
- Ability to travel to assigned cases
- Strong interpersonal and EMR skills
