📋 Description

• Role: Contracted in-home Occupational Therapist for Jukebox Health.
• Conduct in-home assessments and functional evaluations.
• Deliver home modification recommendations and fall prevention guidance.
• Collaborate with clients, families, and Director of Clinical Operations.
• Maintain documentation in our electronic system.

🎯 Requirements

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Experience in home health, aging in place, or disability services preferred.
• Strong interpersonal and clinical reasoning skills.
• Proficiency with technology and digital documentation.
