Contracted In-Home Occupational Therapist

📋 Description

• Conduct in-home assessments and functional evaluations
• Deliver evidence-based home modification recommendations
• Educate clients and caregivers on fall prevention and mobility
• Engage clients and families in collaborative care planning
• Collaborate with Clinical Operations to share findings and improve processes
• Maintain accurate documentation in our electronic system

🎯 Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team
