- Conduct comprehensive assessments and develop individualized care plans
- Coordinate home care services and other supports

🎯 Requirements
- Nursing degree strongly preferred; MSW considered
- Experience in geriatrics or elder care preferred
- Strong interpersonal and communication skills
- Ability to work independently while collaborating with families and service providers
- Valid driver’s license; company car provided for work-related travel
