📋 Description

• Attend a 2-3 week paid training program.
• Meet patients in the community, at home, or in facilities; assess needs.
• Help patients with health-related social issues (homelessness, substance use, hunger).
• Build a directory of community resources with the care team.
• Document patient encounters in our technology systems.
• Meet patients virtually by phone or video as appropriate.

🎯 Requirements

• Highly organized and self-motivated to work independently and manage schedules efficiently.
• Strong interpersonal communication and active listening.
• Cultural competency; able to work with diverse groups.
• Computer literate; experienced with tech for virtual communication, scheduling, and documentation.
• Reside within commutable distance to Greater Seattle.
• Current Driver’s license and access to an insured vehicle.
