Community Health Worker

📋 Description

• Complete initial assessments with patient/family to identify needs.
• Build trust through ongoing rapport with patients.
• Empower patients via motivational interviewing and goal setting.
• Coordinate with interdisciplinary care teams, families, and providers.
• Advocate for patients to ensure needs are addressed by the team.
• Document in EPIC and EMR.
• Assist with social services setup (food, housing, benefits, etc.).
• Understand social determinants of health (SDOH) and their impact.
• Collaborate with Behavioral Health Clinicians as needed for patient care.

🎯 Requirements

• CHW state course completion.
• Bachelor's degree in health-related field or equivalent (preferred).
• Experience with populations with complex medical/behavioral needs.
• Experience in community health or public health.
• Knowledge of Medicaid population.
• Experience documenting in an electronic health record.
• Ability to manage a caseload and assist patient navigation.
• COVID-19 vaccination required.
