Facilitates outpatient mental health and substance use services (individual/family therapy, case mgmt)
Assesses co-occurring and mental health needs using DSM criteria
Completes initial assessments and determines level of care
Reviews records and coordinates with referrals, families, or outside systems
Maintains treatment plans and documents in clinical records
Conducts crisis intervention and participates in multidisciplinary case meetings

🎯 Requirements

Valid, unrestricted NH license as LCMHC, LICSW, or LMFT
Individuals seeking licensure encouraged to apply
Master's degree in Social Work or related field
CISM certification a plus
Experience with Veteran and First Responder populations preferred
