Clinical Support Specialist - 485 Review - Part-Time

📋 Description

• Review and ensure OASIS accuracy for SOC, Recert, and Discharge.
• Verify timeliness and CMS compliance of home health documentation.
• Support Start of Care, Recert, and Discharge OASIS items.
• Provide education to clinicians and clinical managers.
• Maintain current knowledge of Medicare home health rules.
• Collaborate with Home Health Quality Team as needed.

🎯 Requirements

• Active RN license with compact licensure or ability to obtain licensure.
• 3–5 years of home health experience.
• Experience as Clinical Manager or OASIS Reviewer.
• Strong knowledge of OASIS-E, PDGM, and Medicare regs.
• Experience with 485 plan of care reviews and medication reconciliation.
• Experience with Homecare Homebase (HCHB) or similar EMR.
