  • PharmD degree.
  • Licensed pharmacist in Oregon.
  • One of: PGY2 Ambulatory Care Residency; OR PGY1 with ambulatory practice under CPA; OR 3+ years ambulatory practice.
  • Strong communication, organization, and tech skills.
  • Empathy, integrity, and a team-first mindset.
