Partner with physicians and teams to order care plans under a CPA.
Monitor therapy effectiveness and safety; intervene as needed.
Support quality improvement and REMS initiatives.
Advise operations; assist with prior authorizations; educate patients.

🎯 Requirements

PharmD degree.
Licensed pharmacist in Oregon.
One of: PGY2 Ambulatory Care Residency; OR PGY1 with ambulatory practice under CPA; OR 3+ years ambulatory practice.
Strong communication, organization, and tech skills.
Empathy, integrity, and a team-first mindset.
