Clinical Pharmacy Specialist (Hematology/Oncology)

📋 Description

• Conduct disease management assessments under CPAs or clinic protocols.
• On-site work with interdisciplinary teams, including in-person and hybrid care.
• Develop and maintain treatment and supportive care plans.
• Monitor medication regimens for therapeutic goals, adverse effects, and interactions.
• Ensure compliance with FDA standards and national guidelines.
• Identify and prevent medication-related problems; provide counseling.

🎯 Requirements

• PharmD and active Louisiana pharmacist license.
• Oncology Pharmacy Certification (BCOP) within 1 year of hire.
• PGY1+PGY2 residency or 3 years in area of specialty.
• Strong clinical judgment, communication, and process management.
• Knowledge of FDA regulations and national guidelines.
• EPIC and Beacon Oncology experience preferred.
