🎯 Requirements

PharmD required.
Licensed pharmacist in Oregon.
PGY2 Ambulatory Care Residency.
PGY1 Residency + 1 year ambulatory practice under a CPA.
3+ years ambulatory practice with disease-state management.
Strong communication, organization, and tech skills.
Empathy, integrity, and a team-first mindset.
