Clinical Pharmacy Specialist

📋 Description

• Provide medication management in ambulatory care under CPA.
• Partner with physicians and care teams to order plans under CPA.
• Monitor therapy effectiveness and safety; identify optimization opportunities.
• Support quality-improvement and REMS initiatives.
• Provide recommendations to operations for complex prior authorizations.
• Deliver in-person and virtual patient assessments; optimize therapy.

🎯 Requirements

• Hold a Doctor of Pharmacy (PharmD).
• Licensed pharmacist in Oregon.
• PGY2 Ambulatory Care Residency; OR PGY1 + 1+ year ambulatory practice under CPA.
• At least 3 years of ambulatory practice with disease state management.
• Strong communication, organization, and tech skills.
• Empathy, integrity, and a team-first mindset.
