Clinical Pharmacist, Pharmacy Outreach (Remote)

📋 Description

• Counsel patients via telephonic outreach on adherence.
• Address medication issues and provide evidence-based recommendations.
• Perform focused clinical chart reviews to inform interventions.
• Obtain and document patient commitment and address barriers.
• Escalate complex cases to prescribers or interdisciplinary teams.
• Meet and sustain performance goals (STARs, HEDIS).

🎯 Requirements

• PharmD degree.
• Active, unrestricted pharmacist license.
• Multi-state licensure in at least two states.
• Example states: AL, CA, DE, KY, LA, MS, NC, OH, PA, UT, VA, WV.
• Willingness to obtain additional licenses within 6 months.
• 3+ years in value-based care, ambulatory, or community pharmacy.
• In-depth pharmacotherapy knowledge.
• Proficiency with telephony platforms and multiple EHRs.
• Ability to work 8 AM–7 PM EST.
